You will be taking a very positive step when you decide to schedule a 30 minute initial consultation with us. 

At the time of your visit you will be asked to complete an intake form to help us understand what's happening. 

If you've undergone an MRI, CT scan, X-rays or nerve conduction test within the past two years that are associated with your current problem or condition we ask that you bring these films and reports along with you. 

Our clinic staff is ready to assist in obtaining these or transferring any additional medical records that are appropriate.
